Women 50 years and older who are residents of Ontario for longer than 6 months and have no history of breast cancer or implants.
Women 75 and older can not self refer, but can continue in the Ontario Breast Program with a requisiton/referral from their family doctor or patient care provider.
Age Requirements: Ages: 50 year(s) - 74 year(s);

Please bring COVID vaccination information (type and dates) with you to your appointment if applicable.Otherwise no other documents required.
